Will insurance cover the cost of visits? Please call to ensure your coverage.

  1. Call the member services number on the back of your insurance card. Ask the following questions.

  2. Is Rose Mattson Nutrition in-network with my plan? (NPI: 1689154932 and Tax ID 923994694)

  3. Does my plan cover nutrition counseling? (codes 97802 and 97803)

    1. If yes, how many sessions are allowed?

  4. Is Z71.3 (dietary counseling and surveillance) covered, or is a medical diagnosis required (eating disorder, diabetes, IBS etc)?

    1. Please note that MODA, Pacific Source, United Healthcare, & First Choice will likely require a medical diagnosis. Specifically, MODA, First Choice & Pacific Source may only cover visits for an eating disorder or diabetes diagnosis.

  5. Do I have a deductible, copay or coinsurance? If so, what are these costs?

  6. Does my plan require a referral?

  7. Does my plan cover telehealth for nutrition counseling?

  8. Write down the date, name of the person you spoke with, and reference number for your call. This information will be necessary if you ever need to dispute a rejected claim.

What if my insurance does not cover nutrition visits or my insurance denies the claim?

If your insurance does not cover nutrition visits, you have the option of self-pay. You may also request a superbill to submit to insurance for reimbursement. Please talk with Rose for self-pay discounts. If your insurance denies the claim, you will be sent an invoice and are responsible for the remainder of the bill.
